New Patient Information

Please read and completely fill out the new patient forms before your scheduled appointment.  Having paperwork finished upon arrival helps to ensure that you are seen in a timely fashion.


Please remember to bring your current insurance card with you.

To access New Patient forms, click on the link below.  Due to COVID-19 precautions, please complete the forms and email them, along with a photo or copy of the front and back of your medical insurance card to knezap@sbcglobal.net.  If your paperwork and insurance card are not received by the time of your appointment, you will need to reschedule.  Paperwork and insurance information cannot be completed in the office.

COVID-19 Policies

 Important notes during COVID-19

  • Masks are required to enter the office for all patients and parents/caretakers over the age of 2 years, please keep your mask on for the duration of the visit, even if you are alone in the examination room.
  • Appointments will be limited to one parent/caretaker per appointment.  Other family members and guests will not be permitted to enter the office, and our waiting room is closed.
  • You will receive a confirmation call or email the day prior to your appointment.  Please have your insurance card and a credit card prepared for this call, as we will be verifying all information. 
  • We are only accepting credit or debit card/Apple Pay/Google Pay for any in office payments. 
  • No food or drink is permitted in the office with the exception of water and/or formula or breast milk.
  • Please arrive at your scheduled appointment time.  If you arrive earlier than expected, please call the office prior to entering.
  • Everyone entering the office will have their temperature checked prior to entering examination rooms.
  • All examination rooms and equipment will be fully sanitized in between patients, and deep cleaned at the end of each day.

Medical Records

 For a copy of your records please fill out the records release form (link below) and fax or mail it to our office.  We will make every effort to get your records to you or your doctor’s office in a timely fashion, but we require a minimum of 30 days for processing.  Some records are too large to fax, and we will only be able to mail them.  If you have specific instructions or requirements please include this information on the records release form.  For further information or questions please call the office.

Frequently Asked Questions

 How long will my exam last?
A complete eye exam lasts between 60 and 90 minutes.

Will my eyes be dilated?  Will my child's eyes be dilated?
A complete eye exam includes dilation.  This is done usually once a year. If this is your first time seeing us, expect for your eyes to be dilated.

How long will my eyes stay dilated?

Depending on the color pigment of your eyes, normal dilation lasts between 3 and 4 hours. The lighter the pigment, the longer your eyes may be dilated.  You may also want to bring a pair of sunglasses with you to the appointment as your eyes will be more sensitive to sunlight.  

Does my medical insurance cover my exam?
Most medical insurance companies do not pay for examinations that do not have a medical diagnosis.  This includes refractive errors, family history, school exams, and any other normal ocular exam.  For normal exams, payment is expected at the time of the visit. Please contact your insurance for questions concerning coverage.

Does my vision insurance cover my exam?
As medical doctors, we are only contracted with your medical insurance carriers.  Vision plans are designated for opticians and optometrists.  However, if your medical insurance does not cover your "routine exam" please request a detailed receipt in which you can file directly with your vision plan.

Do you validate parking?
We ONLY validate parking at the Northwestern Self Park (Huron Superior Garage) located at 222 E. Huron.  This is payable in our office for $12.00 card only. Validation is good for 4 hours from the time you entered the parking garage.
